Online survey for previous and current SCforH project partners

This short 3-minute survey aims to evaluate the dissemination of Sports Club for Health Guidelines in the European countries.

The survey is conducted as part of the Sports Club for Health 2015-2017 project, funded by the European Commission and supported by the WHO/HEPA Europe network.

You are invited to participate in the survey as a current or previous partner in the Sports Club for Health project, to provide information related to your country.

If you used an identifying token to access this survey, please rest assured that this token will not be stored together with your responses.
